Case Management Redesign (CMRD) updates from The Arc Arapahoe, Douglas & Elbert Counties.

The Arc Arapahoe, Douglas & Elbert Counties (The Arc ADE) is aware of ongoing concerns and problems with the recent transitions related to Case Management Redesign (CMRD), most notably case management shifts between Developmental Pathways and Rocky Mountain Human Services (RMHS). We understand there have been issues related to data entry, continuation of services and changes to individual Case Management Agencies since the transition began. We would like our community of Arapahoe, Douglas and Elbert Counties to know we are working with Developmental Pathways and Rocky Mountain Human Services (RMHS) around these issues. We at The Arc ADE are committed to ensuring our community is supported throughout the Case Management Redesign process. We are collaborating with Developmental Pathways on the ongoing transition plan. The Arc ADE is committed to ensuring our community is well-informed and well-supported before, during, and post-transition timelines.

We are aware that some individuals’ Case Management Agency has changed, and more may see changes in the coming months. This is something we cannot predict but can support people with correcting an unexpected change. If you or someone you care for are interested in using a different Case Management Agency than the one you are currently receiving supports from or your Case Management Agency was changed without your request, please reach out to The Arc ADE. We can support you in understanding your options and help (if needed) with contacting your current Case Management Agency or case manager.

The Arc ADE is here and wants you all to know, you are not alone. We are aware of the problems and will do our best to support people through this change.
